What is Case Management?
Case management is a term used to describe the supports and services provided to individuals living with mental problems or illness to help them achieve their personal recovery goals. Case management services include:
- Comprehensive individualized assessment
- Client-directed goal planning
- Assistance with problems of daily living
- Client advocacy
- Supportive counselling
- Family support and education
- Linkage to services and resources
- Monitoring of service delivery
- Service coordination

Hospital Liaison
Referrals to this program are received from Hotel Dieu, Providence Continuing Care, and Quinte Health Care only, for individuals who wish to return to Hastings and Prince Edward Counties
Mental health case management services are provided immediately upon discharge from hospital to assist in the transition from hospital to community living. After liaison, client is referred to regular case management.
